PDi Exercise Scholarship 

The PDi Exercise Training Scholarship Program will provide 6 months of aerobic exercise funds worth $100 per month to fund 4 people with Parkinson disease to help prevent motor decline while increasing overall quality of life.


The PDi will cover costs up to $100 per month for 6-months of aerobic exercise with a minimum of 8 sessions per month lasting at least 30-minutes per session. Group classes or individual private instruction are allowed. Such exercise must be rooted in evidence-based outcomes which may include (but not limited to) Tai Chi, Rock Steady
Boxing, Zumba, Punching Out Parkinsons, Yoga, PWR!Moves, etc,

Application Submission Requirements

I. A treating physician must provide a letter of approval accompanying the application that
includes the following information:

-Type of exercise program (while sitting, standing, with or without a mobility use aid, etc.)

that would most safely benefit their patient.

-Diagnosis Using the Hoehn & Yahr Parkinson Disease Eligibility Criteria below:

o Prodromal, young onset, or newly diagnosed
o Early stage
o Moderate stage without mobility aid use
o Moderate stage with mobility aid use

-Fall Risk (High, Moderate, Low)

-Letters must be signed and dated by the physician


II. The applicant must provide 250 words or less explaining:
o their interest or need in applying for the scholarship, how they plan to use the scholarship
funds such as Purchase of classes / instruction and/or Transportation to and from aerobic
exercise classes, etc., or other reasons for applying.
Note:
-Psychosocial activities paired with physical activity are especially preferred to help improve overall
quality of life.
-Participants may choose whatever program they wish as long as it is rooted in evidence-based
outcomes, aligns with their physician’s letter of approval, and aligns with the scholarship
requirements.

Indemnification Clause: The Participant and any of their assigns agree to indemnify, defend,and hold harmless the Parkinson Discovery Institute (PDi), its executive director, board ofdirectors, agents, and affiliates from and against any and all claims, damages, liabilities, losses,costs, expenses, including reasonable attorney’s fees, arising from or in connection with theParticipant’s participation in the aerobic exercise program funded by the PDi.

Applicant Notification
Applicants will be notified by email on the selection by Monday, March 9, 2026.


Release of Funds:
We will release funds for the first 3-months in the form of a $300 check made out to the
applicant. At the end of these first 3-months, attendance records signed by the exercise
facilitators / teachers and 3-4 sentences on obtained benefits (if any) is required prior to
the second release of $300 for the next 3-months of exercise participation.
